Sudden Cardiac Arrest and Concussion Awareness Forms

Club Officials, Managers and Coaches,

Starting the 2015-2016 school year, schools will be requiring any clubs/teams that use school district facilities to sign a document stating that the club/team is in compliance with the Concussion and Sudden Cardiac Arrest legislation

This Legislation went into effect on July 24, 2015.   This legislation is similar to the Zackery Lystedt Lawy(House Bill 1824) which addresses concussions that went into law in 2009.    The Sudden Cardiac Arrest legislation immediately affects clubs and team that use school district facilities but all member clubs and teams participating in the PSPL must comply with the new legislation.

Below are the steps that all clubs/teams must take:

  • Every year before the start of the season all clubs/teams that participate in the PSPL must have all their players and parents/guardians sign a form indicating they have read the information on Cardiac Arrest. Pamphlet of information about Cardiac Arrest.     
  • Club and teams that participate in the PSPL must have all their coaches and assistant coaches complete an online Sudden Cardiac Arrest prevention program and present a certificate of completion to their club/team prior to the start of the 2015-2016 season. https://nfhslearn.com/courses/61032. The training takes approximately 10 to 15 minutes and a certificate is issued upon completion.   This training must be done every three (3) years.
