Misconduct Report for Referees

Directions for Submittal of the Misconduct Report

Submit a separate form for each caution/send off/warning/dismissal within 48 hours of match completion.  If your Referree Assoc.  has online misconduct reporting via Ridgestar Misconduct Project please ensure the "Authority" listed for the report is the PSPL.  If you are not using Ridgestar or have no online reporting system available please use the linked forms below for report submittal.

Instructions:
Use one form per caution or send-off issued. Complete the form entirely. Refer to the USSF Referee Administrative Handbook for sections entitled "Procedure: Guide to Report Writing and " Summary of Report Writing Procedures" for assistance in completing this report. File this report online within forty-eight (48) hours after the match.
